Shadyside Athletic Physicals 2022-2023 School Year
The Shadyside Athletic Department, partnered with Wheeling Hospital, will offer athletic physicals for school year 2022-2023 on Tuesday June 14, at 5:30, for students entering grades 7-12. This is for all sports for the entire school year. This event will be held at Wheeling Hospital Family Medicine Center, Tower 4, 4th floor, Suite 401. For those students wishing to take part, there will be a $10 fee associated with this. Checks can be made payable to The Family Medicine Center at Wheeling Hospital. Physicals will not be provided without the fee being paid. Students will need to bring with them a physical form with parent signatures already on them. These can be picked up in the high school office, or printed from the OHSAA website. Just click on SPORTS MEDICINE, scroll down to PRE-PARTICIPATION PHYSICAL EXAM FORM.
